Out-of-hospital cardiac arrest (OHCA) is responsible for nearly 34,000 deaths annually in France.1 Despite advancement in cardiopulmonary resuscitation (CPR), survival rates at hospital discharge remain low.2 Most OHCAs are of cardiac origin, with acute coronary syndrome (ACS) being the leading cause.3 Refractory cardiac arrest is defined as the inability to achieve return of spontaneous circulation (ROSC) despite 20โ€“30 min of state-of-the-art CPR.4 Beyond this timeframe, the chances of survival with good neurological outcomes drop significantly.

Association of hyperosmolar therapy with cerebral oxygen extraction after cardiac arrest

Elevated jugular bulb venous oxygen saturation (SjvO2) after cardiac arrest may be due to diffusion-limited oxygen extraction secondary to perivascular edema. Treatment with hyperosmolar solution (HTS) may decrease this edema and thus the barrier to oxygen diffusion. Alternatively, SjvO2 may rise when cerebral metabolic rate declines due to irreversible cellular injury, which would not be affected by HTS. Electroencephalography (EEG) may differentiate between these etiologies of elevated SjvO2. We hypothesized SjvO2 would be lower after treatment with HTS and EEG could identify treatment responders.
