💹የስኳር በሽታ💹

✅የስኳር በሽታ ሥር የሰደደ የሜታቦሊክ በሽታ ሲሆን፣ በደም ውስጥ ያለው የስኳር መጠን ከፍ ያለ ሲሆን ይህም ከጊዜ ወደ ጊዜ በልብ, በደም ሥሮች, በአይን, በኩላሊት እና በነርቭ ላይ ከፍተኛ ጉዳት ያስከትላል.

✅ባለፉት ጥቂት አሥርተ ዓመታት ውስጥ  የስኳር በሽታ እና ስርጭት ከጊዜ ወደ ጊዜ እየጨመረ መጥቷል.

✅እንደ አለም አቀፍ የስኳር ፌደሬሽን ዘገባ እ.ኤ.አ. በ 2021 ፣በግምት 537 ሚሊዮን ጎልማሶች (ከ20-79 ዓመታት) በስኳር ህመም ይኖራሉ ፤ እና በ 2030 አጠቃላይ የስኳር ህመምተኞች ቁጥር ወደ 643 ሚሊዮን እና በ 2045 ወደ 783 ሚሊዮን ከፍ ይላል ተብሎ ይገመታል። ከእነዚህ  ውስጥ የአዋቂዎች ከስኳር በሽታ ጋር የሚኖሩ አብዛኛዎቹ (ከ4 ውስጥ 3) ዝቅተኛ እና መካከለኛ ገቢ ባላቸው አገሮች ውስጥ ነው፡፡

✅በዚሁ ዘገባ በአፍሪካ አህጉር  ውስጥ የስኳር ህመም እና የግሉኮስ መቻቻል ችግር ያለባቸው ሰዎች ቁጥራቸው እየጨመረ ሲሆን በ 2045 እ.ኤ.አ 117 ሚሊዮን ይደርሳል ።ያልታወቀ የስኳር በሽታ መጠን በአፍሪካ ክልል ከፍተኛው ነው(54%)።

✅በጣም የተለመደው የስኳር በሽታ ሁለት ሲሆን፡-
1. ዓይነት 2 በመባል ይታወቃል። ብዙውን ጊዜ በአዋቂዎች ላይ የሚከሰት ሲሆን፣ ሰውነታችን ኢንሱሊንን መቋቋም ሲችል ወይም በቂ ኢንሱሊን በማይፈጥርበት ጊዜ ነው።

2. ዓይነት 1 የስኳር በሽታ በተሟላ የኢንሱሊን ምርት እጥረት የሚታወቅ ሲሆን በየቀኑ የኢንሱሊን አስተዳደርን ይፈልጋል ።

✅በእርግዝና ወቅት ተለይቶ የሚታወቅ የስኳር በሽታ ክፍል የእርግዝና የስኳር በሽታ ተብሎ የሚጠራ ሲሆን ይህም ሴቶች በእርግዝና ወቅት ለችግር ተጋላጭነት, ለወሊድ እና ለወደፊቱ ዓይነት 2 የስኳር በሽታ የመጋለጥ እድላቸው ከፍተኛ ነው።

የስኳር በሽታ ምልክቶች በድንገት ሊከሰቱ ይችላሉ ፣ ወይም ቀላል ሊሆኑ እና ለመታወቅ ብዙ ዓመታት ሊወስዱ ይችላሉ።

✅የስኳር በሽታ ምልክቶች የሚከተሉትን ያካትታሉ:

የማንኮራፋት ችግር | Snoring disorder

የማንኮራፍት ችግር ማህበራዊ ጫና ከመፍጠሩ ባሻገር ጤናላይ የሚያመጣዉ መዘዝ የጎላ ነው።
የማንኮራፋት ችግር የሚከሠተዉ አየር ከሳንባ ወይም ወደ ሳንባ በሚወጣበት እና በሚገባበት ጊዜ በሚያልፍባቸው የአየር ቱቦ በቂ የሆነ ክፍተት ካላገኘ ወይም የሚያልፍበትን የተመቻቸ መስመር የሚያስቀይር ችግር ካጋጠመው የሚፈጥረው የተዛባ ድምጽ ነው።
ይህን የማንኮራፋት ችግር የሚያስከትሉ ምክነያቶች በህጻናት እና አዋቂዎች ላይ የተለያዩ ቢሆንም የሚከሠቱበትን ቦታ በ 4 ከፍሎ ማየት ይቻላል።

1. ከአፍንጫ እስከ ላንቃ ያለው የአየር መተላለፊያ ሲሆን ህጻናት ላይ በአብዛኛው አዴኖይድ የምንለው የቶንሲል ክፍል መጠን መጨመርና በአፍንጫ የሚያልፈውን አየር ሲዘጋ የሚከሠት ነው። አዋቂወች ላይ ከዚህ ቦታ በሚነሳ ችግር ማንኮራፋት ከተከሠተ ካንሠርን ጨምሮ ሌሎች እብጠቶችን ማሠብ እና ምርመራ ማድረግ ግድ ይላል።

2. ከምላስ የኋለኛው ክፍል እና በጎኑ ከሚገኙ ቶንሲሎች ማበጥ ጋር ተያይዞ የሚከሠት ማንኮራፍት ነው። የዚህ ከምላስ ጎን የሚገኘው ቶንሲል የማንኮራፍት ችግር ከመፍጠሩ በላይ በተደጋጋሚ የቫይረስ እና የባክቴሪያ ኢንፌክሽን በመፍጠር ተጨማሪ የጤና እክል ያስከትላል።

3. በአፍና መንገጭላ አካባቢ የሚከሠቱ የአፈጣጠር ችግሮች እንዲሁም ከነዚህ ቦታወች ተነስተው የኅለኛውን የአየር መተላላፊያ ሊዘጉ የሚችሉ እብጠቶች እንደ ምከንያት ይጠቀሳሉ።

4. የኋለኛው የአየርና የምግብ የጋራ መተላለፊያ ግድግዳን የሚሠሩ ጡንቻወች መዛል ፣ በስብ መሞላት ፣ በእብጠት መጠቃት ወይም መግል መቋጠር ከተፈጠረ የሚከሠት ይሆናል።

ማንኮራፍት የሚከሠትባቸውን ምክነያቶች በወፍ ዘረር ካየን የዚህ ችግር መጠን ከሠው ሠው እንደተፈጠረው የአየር መተላለፊያ መስመር መጥበብ መጠን ይለያያል።

ይሔም ከልማዳዊ ማንኮራፋት እስከ ሳንባ ከረጢቶች መጠን መቀነስ ብሎም የሳንባ ደም ስሮች ግፊት እና የልብ ችግር ሊፈጥር ይችላል።

በተጨማሪም አካላዊና አእምሯዊ የእድገት ችግር ፣ የጸባይ ችግር (በተለይም ህጻናት ላይ) ፣ ቀን አብዝቶ የመተኛት ችግር ፣በስራ የመዛልና ውጤታማ ያለመሆን እና የመሳሠሉ ተጽኖወችን ይፈጥራል።

በትዳር አጋር ላይ የሚፈጥረው ጫና እና ማህበራዊ ቀውሡም ሌላው ጉዳቱ ነው። ማንኮራፍት የፈጠረውን የጤና ችግርደረጃ ለማወቅ አይነተኛ የሚባለው መሣሪያ (polysomnography) ሲሆን የሚፈጠሩ ስሜቶችን እና ምልክቶችን በማየት እናየልብ ፣ የአተነፋፈስ ምርመራወችን በማድረግ ደረጃውንማወቅ ይቻላል። የዚህን ጀረጃ ማወቁ ጥቅሙ ለእያንዳንዱ ደረጃ የሚሠጠው ህክምና የተለያየ መሆኑ ነዉ።

ወደ መፍትሔው ስንመጣ ፤ መፍትሔው የሚወሠነው ማንኮራፍቱን በፈጠረው ችግር እና የችግሩ መጠን ሲሆን

1. በአብዛኞች ህጻናት ላይ ከአፍንጫ ጀርባ እና ከምላስ ጎንያለውን ቶንሲል በሠርጀሪ በማስወገድ የሚስተካከል ይሆናል።

2. በአዋቂወች ደግሞ እባጮች (እጢወች) እንደሌሉ ከተረጋገጠ በኋላ ክብደት መቀነስ ፣ የኦክስጅን ህክምና (CPAP) ብሎም የአየር መተላለፊያን ቱቦ ለማሥፍት የሚደረጉ ሠርጀሪወች እና ሌሎች ዘርፈ ብዙ ሠርጀሪወች) የህክምና አማራጮች ናቸው።

ስለዚህ፦ ማንኮራፋትን እንደ ቀላል ችግር በማየት አንዘናጋ፤ቢያንስ ያለንበት የማንኮራፍት ጀረጃ በምን መሥተካከል እንደሚችል ሀኪሞችን እናማክር።

✅22 ሚሊዮን የሚገመት ቁጥር ያላቸው እድሜያቸው ከ 15 አስከ 49 የሆነ ሴቶች በየ አመቱ ይያዛሉ፣ ከነዚህ ዉስጥም 7095 ኬዞች 4732 የሚሆኑትን ለሞት ይዳረጋሉ ። በ ሀገሪቱ በ ገዳይነቱ እና ብዙ ህዝብ በማጥቃት ሁለተኛ ደረጃን ይዟል ። የ ማህፀን ጫፍ ቅድመ ካንሰር ምርመራ አንደኛው የ ማህፀን ጫፍ ካንሰርን መከላከያ መንገድ ቢሆንም ፤ ምርመራውን የሚያደርጉ ሴቶች ቁጥር እጅግ አናሳ ነው።

ምርመራው በ ሆስፒታላችን 24 ሰዓት የሚሰራ ሲሆን ውጤቱም ከ ሁለት ቀን ባልበለጠ ጊዜ ውስጥ ይደርሳል። ምርመራው በ ሶስት ደቂቃዎች ውስጥ ይወሰዳል።

✳️ከምርመራ በፊት የሚደረጉ ቅድመ ዝግጅቶች ምንድናቸው ?

1)የ ወር አበባ ላይ ላለች ሴት ምርመራው አይሰራም። 2) ከ ምርመራው 24 ሰዓት አስቀድሞ ከ ግብረ ስጋ ግንኙነት ፣ ብልትን ከመታጠብ እንዲሁም በ ብልት ውስጥ ከሚደረጉ ማናቸውም አይነት መድሐኒቶች ወይም ክሬሞች መቆጠብ ያሻል።

✳️የማህፀን ጫፍ ቅድመ ካንሰር ምርመራ ምንድነው ?

✅ቅድመ ምርመራ ማለት አንድ ህመም ምልክት ከማሳየቱ በፊት መመርመር ማለት ሲሆን ፣ የማህጸን ጫፍ ቅድመ ካንሰር ምርመራም ከነዚህ ምርመራዎች መካከል ይገኝበታል።

✳️ለምን ይጠቅማል ?

✅በ ማህፀን ጫፍ ላይ የሚታይ የቅድመ ካንሰር ለውጥ ወደ ካንሰር ለመቀየር ከ ሶስት እስከ ሰባት አመታትን የሚፈጅ ስለሆነ በ ቅድመ ካንሰር ምርመራ የ ህመም ምልክት ሳይታይ ይደረስበታል ።

ከ 21 አመት ጀምሮ በ የ ሶስት አመቱ ይደረጋል። ነገር ግን ከዚህ በታች የተዘረዘሩት አጋላጭ ሁኔታዎች ካሉ ከ ሶስት አመት ባነሰ ጊዜ ውስጥም ምርመራውን ማድረግ ያስፈልጋል፦

1) የ ሰውነት የመከላከል አቅም በተለያዩ ምክንያቶች የተዳከመ ከሆነ ለ ምሳሌ ኤች አይ ቪ ያለባቸው ህሙማን
2) በቅርብ ጊዜ የተሰራ ምርመራ ወይም ናሙና ቅድመ ካንሰር ጠቋሚ ከሆነ ።

✳️ምርመራውን መቼ ማቆም ይቻላል ?

✅ከ 65 አመት በኋላ ከዚህ በታች የተዘረዘሩት መስፈርቶች ከተሟሉ ምርመራውን ማቆም ይቻላል፦
1) ከዚህ በፊት ምንም አይነት የ ማህጸን ጫፍ ቅድመ ካንሰርም ሆነ የ ካንሰር ህመም ታሪክ ከሌለ።
2) ሶስት ከማህጸን ጫፍ ነጻ የሆኑ ተከታታይ የቅድመ ካንሰር ምርመራ ውጤቶች በ አስር አመት ውስጥ የመጨረሻው ውጤት ሶስት አመት ያላለፈው ከሆነ።
