Urine culture is the appropriate test for asymptomatic bacteriuria, if screening is indicated.
The Infectious Diseases Society of America (IDSA) and the European Association of Urology (EAU) definitions for asymptomatic bacteriuria:
For asymptomatic female adults: bacterial growth ≥ 105 colony-forming units (CFU) / ML isolated from 2 consecutive voided urine specimens
For asymptomatic male adults: bacterial growth ≥ 105 CFU/mL isolated from a single voided urine specimen
For asymptomatic adults with catheters: bacterial growth ≥ 105 CFU/mL isolated from a single catheterized urine specimen

When to Treat Asymptomatic Bacteriuria (ASB)? 🩺
في حالة وجود WBCs في تحليل البول فوق المعدل الطبيعي بدون وجود أعراض UTI لا نكتب للمريض Antibiotic الا في حالات استثنائيه :
✅ Treatment is Recommended in ONLY Two Cases:
1️⃣ Pregnant Women (Strong Recommendation)
Screen early in pregnancy and treat to prevent complications like pyelonephritis, preterm labor, and low birth weight.
Antibiotics should be chosen based on culture sensitivity and pregnancy safety.
2️⃣ Patients Undergoing Invasive Urologic Procedures with Potential for Mucosal Trauma (Strong Recommendation)
Screen and treat before the procedure to prevent postoperative infections.
Start antibiotics 30-60 minutes before the procedure.
Short-course therapy (1-2 doses) is usually sufficient.

❌ DO NOT Treat in the Following Cases:
🚫 Nonpregnant adults, including older adults
🚫 Children
🚫 Patients undergoing non-urologic surgery
🚫 Renal transplant recipients (if surgery was >1 month ago)
🚫 Patients with well-controlled diabetes
🚫 Patients with indwelling catheters (unless symptomatic or just after removal)
🚫 Spinal cord injury patients
🚫 Patients with pyuria alone (without symptoms)
