বিগত কুড়ি নভেম্বর বুধবার গেল বিশ্ব সিওপিডি দিবস। এই সিওপিটি (COPD)বা ক্রনিক Obstructive pulmonary ডিজিজ হল আমাদের শ্বাসনালীর অবস্ট্রাকশন বা সংকোচন জনিত সমস্যা। এছাড়াও এরকম রোগ হলো হাঁপানি বা অ্যাজমা ও broncheactasis। এই COPD রোগটি পৃথিবীর প্রথম তিনটি মৃত্যুর কারণ এর মধ্যে একটি এবং সেটি হার্ট অ্যাটাক (IHD) ও স্ট্রোকের পরেই এর স্থান। প্রতিবছর এই রোগে পৃথিবীতে প্রায় ৩০ লক্ষ লোকের মৃত্যু হয়।
তো প্রথমেই আমরা জেনে নেব এই রোগের লক্ষণ কি ?
প্রধানত চার রকমের লক্ষণ দেখা যায় তার মধ্যে প্রথম হলো শ্বাসকষ্ট, এই শ্বাসকষ্ট একটু পরিশ্রম করলেই বাড়তে থাকে প্রথমদিকে সিড়ি ওঠানামা করলে ও পড়ে সামান্য হাঁটাচলা করলে, ও শেষের দিকে প্রায় সবসময়ই শ্বাসকষ্ট হয়। দ্বিতীয়তঃ প্রচুর পরিমাণে কাশি ও কাশির সাথে কফ বেরোনো। এর লক্ষণ শুরু হয় ভোরবেলা ঘুম থেকে উঠেই অনেকটা কফ বেরোনো দিয়ে। তৃতীয়ত অনেকের বুকে চাপ ধরে থাকে। চতুর্থত অনেকের এই সব রকম কষ্টের বারবার অ্যাটাক হয় ও হসপিটালে ভর্তি হতে হয়।

এবার জেনে নিই এ রোগের রিক্স ফ্যাক্টর বা কেন হয় ?
এই রোগ জিন (Gene)পরিবেশ (Environment)ও টাইম(time )এই তিনটের মিথস্ক্রিয়া বা interaction এ হয় । এই রোগের কারণ হলো ধূমপান বা smoking, এবং বিষাক্ত ধোঁয়া, সেটা বাড়ির উনুনের হোক বা বাইরের দূষণের হোক। প্রধানত যারা chain smoker এবং যারা 20 প্যাক years বা দিনে এক প্যাকেট করে কম করে কুড়ি বছর সিগারেট খেয়েছে তাদের এই রোগ হওয়ার সম্ভাবনা খুব বেশি। এবং যারা বিড়ি খায় তারা যদি এভাবে হিসেব করে যে তাদের প্রতিদিন বিড়ি খাওয়ার সংখ্যা ও যত বছর বিড়ি খেয়েছে সেটা দিয়ে গুন করলে তিনশো এর বেশি হয় ( Smoking Index - 300) ,তাহলে তারাও বিপদ সীমাতে আছে। এছাড়া বাড়ির মহিলারা যারা অনেক বছর ধরে কয়লার বা ঘুটের উনান এ রান্না করছে এবং উনান এর ধোঁয়ায় কুড়ি বছরের ওপর কাটিয়ে দিয়েছে তাদেরও এই রোগ হওয়ার সম্ভাবনা খুব বেশি। কিছু কিছু ক্ষেত্রে জিনের সমস্যার জন্য ( alpha 1 antitrypsin deficiency)কম বয়সে COPD রোগ হতে পারে।
তাছাড়া এখন দিল্লিতে যেমন দূষণ হয়েছে, সেরকম আবহাওয়ায় 20 বছর কাটালে এই রোগ কিন্তু হানা দিতে পারে।
তাহলে কেমন করে এ রোগ হয়েছে বুঝবো? সেটা জানতে হলে প্রথমেই বলি এই বছর বিশ্ব সিওপিডি দিবস এর থিম হলো " know your lung function" । এই lung function বা ফুসফুসের কার্যকারিতা জানা যায় স্পাইরোমেট্রি (Spirometry) টেস্টের মাধ্যমে। এই রোগে ফুসফুসের কার্যকারিতা স্থায়ীভাবে ( irreversiblely) কমে যায় এবং সেটা বাড়তে থাকে যদি ট্রিটমেন্ট না করা হয় এমনকি কখনোই সেটা আবার পুরো আগের অবস্থায় ফিরে আসে না। এই রোগ হতে গেলে ফুসফুসের শ্বাস নালি গুলোকে ধোঁয়ার মাধ্যমে অনেকটা ক্ষতিগ্রস্ত হতে হয় তাই এই রোগ সাধারণত 40 বছর বয়সের আগে হয় না, তবে একবার হয়ে গেলে আর সম্পূর্ণভাবে সেরে যাওয়া সম্ভব নয়। যদি কারো আগে বলা লক্ষণগুলো দেখা যাচ্ছে এবং তাদের বয়স 40 বছরের বেশি তাহলে অবশ্যই আপনার ফুসফুসের কার্যকারিতা পরীক্ষা বা spirometry test করে দেখুন।

প্রতিকারের উপায় !? সবার প্রথমে ধূমপান করা ছাড়তে হবে। যদি ধূমপান ছাড়তে সমস্যা হয় তবে de-addiction clinic বা সাইক্রিয়াটিস্ট বা বক্ষরোগ বিশেষজ্ঞ দেখাতে পারেন। যে সমস্ত বাড়িতে এখনো কয়লা দিয়ে রান্না হয় তাদের গ্যাস স্টোভ ব্যবহার করতে হবে। এবং সর্বোপরি আমাদের পরিবেশে দূষণ কমানোর চেষ্টা করতে হবে। একবার এরকম হয়ে গেলে তার ট্রিটমেন্ট বিভিন্ন ইনহেলার জাতীয় ওষুধ দিয়ে করা হয়। এই ইনহেলার গুলো চুপসে যাওয়া শ্বাসনালী গুলোকে খুলে রাখতে সাহায্য করে তাই শ্বাসকষ্ট এর সমস্যা কমে যায়। এখন অনেক নতুন ইনহেলার এসেছে যাতে ২৪ ঘন্টা এই শ্বাসনালী খোলা রাখার ব্যবস্থা করা সম্ভব। যেহেতু এই রোগে বারবার ফুসফুসে ইনফেকশন হয় তাই COPD রোগীদের কিছু ভ্যাকসিনেশন নেওয়া উচিত,বিশেষত নিউমোনিয়া ও ইনফ্লুয়েঞ্জা ভ্যাকসিন।

সিওপিডি একটি সাধারণ, প্রতিরোধযোগ্য এবং চিকিত্সাযোগ্য রোগ, তবে ব্যাপকভাবে দেরিতে রোগ নির্ণয় এবং ভুল রোগ নির্ণয়ের ফলে রোগীদের কোনো চিকিৎসা হয় না বা ভুল চিকিৎসা হয় । উপযুক্ত সময়ে COPD এর নির্ণয় ও তার চিকিৎসা জনস্বাস্থ্যের উপর খুব গুরুত্বপূর্ণ প্রভাব ফেলতে পারে।

Answer - it's called Palmoplantererythrodysesthesia.

Cause - Capecitabine Drug adverse effects.( This patient taking that drug for stomach adenocarcinoma NOS)

Treatment - Avoid friction
Apply Vaselin thrice daily over palm and sole x 10 days
Urea Cream is to be applied locally thrice daily x cont.
Tab Pyridoxine 40 mg 2 tabs BD x cont
Hold Tab Capecitabine ( to be decided depends on option available and judgement of oncologist)

Please give your VIEWS on COPD and Spirometry ?. What practice in your country is following ? How do you diagnose them ? Do you use spirometry every time for follow up or only during diagnosis or in between ?

Thought Spirometry is an Objective Test and reproducible by a well trained person..

Part -2 ; GOLD only uses it for simplicity and also spirometry is difficult to perform by the patient and not every time their efforts are the same…

So I think, in a few years or updates this spirometry mandatory will be changed or reconsidered by Gold guidelines.
