H - Hypercalcemia / Hypertriglyceridemia
E - ERCP (Endoscopic Retrograde Cholangiopancreatography)

🔹Clinical features:

💎 General:

Fever, anorexia, tachycardia

💎 Abdominal:

acute severe epigastric abd pain radiating to the back

N/V, jaundice if edema of pamcreatic head

If retroperitoneal hemorrhage:

Cullen sign, grey turner sign

🔺Complications:

▪️Local:

Necrosis
Pancreatic pseudocyst

▪️Systemic:

👀 Retinopathy
🫁 ARDS, alveolitis, pneumonitis
🫀 Heart failure

🩺Ranson criteria:

At Admission (5 criteria):
These are assessed upon patient presentation:
Age: >55 years.
White Blood Cell (WBC) Count: >16,000/mm³.
Blood Glucose: >200 mg/dL (11.1 mmol/L) in a non-diabetic patient.
Serum LDH (Lactate Dehydrogenase): >350 IU/L.
AST (Aspartate Aminotransferase): >250 IU/L.


🩺BISAB criteria:

Blood Urea Nitrogen (BUN):
BUN > 25 mg/dL (or >9 mmol/L).
Impaired Mental Status:
Glasgow Coma Scale (GCS) < 15.

Systemic Inflammatory Response Syndrome (SIRS):

Presence of ≥2 of the following:

Temperature >38°C or <36°C.
Heart rate >90 beats/min.
Respiratory rate >20 breaths/min or PaCO₂ <32 mmHg.
WBC count >12,000/mm³, <4,000/mm³, or >10% immature bands.

Age:
Age > 60 years.
Pleural Effusion:



🔹DDx:
🔹Dx:

• Amylase and/or lipase 3 times or more above normal limit
• Characteristic abd pain
• ACT

🔹Mx:

🔺Emergency:

Admission, NPO, NG tube, analgesic e.g. pethidine
O2

C:

• 2 wide bore canulae:

a) Inx

IV fluid e.g. RL 200-400 ml/hr
IV electrolyte correction

then AACRO 👇🏾

IV antibiotics (ceftazidime, cefoperazone, cefotaxime) 
imipenem
Anticholinergic
TPN
Calcium gluconate 10 ml 10% 8th hourly "bcause most patients are hypocalcemic"
Ranitidine IV 50 mg 8th hourly
Octreotide 50 ug bolus then 50 ug in 5% dextrose.


• Urinary catheter: 50 ml/hourly
• ECG monitor


💡Risk assessment

A 24-year-old married nulliparous has an 18-month history of cyclic, debilitating pelvic pain related to menses which are regular and heavy requiring 10-15 thick pads on the days of heaviest flow. For the past year, the patient has been engaging in regular intercourse without contraception in an attempt to conceive. Pelvic examination showed normal-size, immobile, retroverted uterus with nodularity and tenderness on palpation of the uterosacral ligaments.

🌸Cholangitis is suggested by the presence of the Charcot triad:👉🏻 fever, jaundice, and pain in the right upper quadrant.
🌸In patients with suppurative
cholangitis who fail to respond to intravenous antibiotics and fluid resuscitation, the nonoperative approach is the preferred intervention via either percutaneous or endoscopic drainage of the obstructed common bile duct.
🌸If endoscopic retrograde cholangiopancreatography (ERCP) or percutaneous transhepatic biliary drainage (PTBD) fails, surgery is indicated.
🌸This is usually best accomplished by
surgical placement of a T tube into the duct.
🌸Cholecystostomy will be effective only if there is free flow of bile into the gallbladder via the cystic duct and in general should not be depended on to secure drainage of the common bile duct.
🌸A cholecystectomy would not provide drainage of the obstructed common bile duct.

في حاجات بتكتسبيها بالخبرة من بوصات كتيرة على حسب الاعتادو عليهو ودا بيكون افضل من ال guideline وفي حاجات بتكتشفيها براك على حسب العيانين الكتار ( الخبرة)

قبل 4 سنة واحد من دكاترتنا قال لي الحقيقة المرة ي عبدالرحمن مرات كتيرة ح تلقى انك بتتعلم براك فأندرش ف المستشفيات م تقع ف الوقعنا فيهو نحن . ما معانا زول وبنطبز ف العيانين قال لي البلد دي تعليمها ف القطاع الطبي ردئ 💔 (للامانة هو حفار ومتخرج بمرتبة الشرف وامتحن OET ومشى نيوزلندا . وانا من بعد كلامو ومن وقتها كنت بقزقز ف المستشفيات وشكرا ليهو جدا اينما كان

الفرق في انك دخلت مستشفيات بدري بصنع شخصيتك حتى لو م كنت حفار

الصنع شخصيتو ف المحلات الاشتغل فيها ثباتو ف المواقف واعادة عيان كان اهلو قنعو منو للحياة بسبب انو من بدري داقش المستشفيات . دكتور ابراهيم برغم انو كان حفار وفي رابطة جامعة الرباط لما نزل الامتياز بقى رئيسهم بسبب معرفتو وثباتو اكتر من الباقين فالمعرفة غير دراسة الجامعة عايزة زول دخل المستشفى وال ER كتير .
