Investigations are driven by the cardiovascular status of the patient.

In torso trauma, the best and most sensitive modality is a CT scan
with intravenous contrast for blunt injury if the patient is stable.

Routine investigation in the emergency department of injury the chest is based on clinical examination, supplemented by chest radiography.

Ultrasound can be used to differentiate between contusion and the actual presence of blood.
Extended focused assessment with sonar for trauma (eFAST) is

Correct Answer - D
Answer: D. During feeding

Congenital hypertrophic pyloric stenosis

#Presentation
Vomiting is the presenting symptom (child vomits milk and no bile is present)

Immediately after vomiting the child is hungry i.e. loss of appetite
does not occur.
Weight loss is striking and rapidly the infant becomes emaciated and
dehydrated.

However, greater awareness of pyloric stenosis has led to earlier
identification of patients and hence with fewer incidences of chronic
malnutrition and severe dehydration - Nelson 10th/1130

The diagnosis is usually made with a test feed: In this, the baby
is fed with the bottle by a nurse or mother and surgeon :

IHPS/CHPS can be diagnosed clinically. During a test feed, there is visible gastric peristalsis passing from left to right across the upper
abdomen and in a relaxed baby, the pyloric 'tumor' is palpable as an
'olive' in the right upper quadrant.
The diagnosis can be confirmed by an ultrasound, which shows the thickened pyloric muscle.

Correct Answer - B
Ans. B: Sapheno-femoral Incompetence

A test to assess the competence of the saphenofemoral junction.

The Brodie-Trendelenburg test is used to detect venous
incompetence and to differentiate between perforator and GSV
incompetence.

The Brodie-Trendelenburg test is highly sensitive for the
identification of superficial and perforator reflux.

SFJ (saphenofemoral junction) incompetence is diagnosed if the
distal veins fill rapidly upon release of the tourniquet.

Some textbooks refer to the Trendelenburg test and the tourniquet test interchangeably

Correct Answer - C
Ans. is 'c' i.e., Staphylococcus
Granulomatous mastitis

* Granulomatous inflammation is present in less than 1% of all
breast biopsy specimens.

* The causes include systemic granulomatous disease (e.g.,
Wegener granulomatosis or sarcoidosis) that occasionally involve the breast, and granulomatous infections caused
by mycobacteria or fungi.

* Infections of this type are most common in immunocompromised
patients or adjacent to foreign objects such as breast prostheses or nipple piercings.

* Granulomatous lobular mastitis is an uncommon breast-limited
disease that only occurs in parous women.

* The granulomatous inflammation is confined to the lobules,
suggesting that it is caused by a hypersensitivity reaction to antigens expressed by lobular epithelium during lactation.

Correct Answer - A
Answer- A. HGF
Hepatocyte growth factor (HGF/scatter factor) levels rise to high
levels soon after partial hepatectomy. This is the only factor tested
that acts by itself as a potent mitogen for isolated hepatocytes
cultured in vitro. This factor is also of critical importance in
development of the liver, as target deletions of its gene lead to fetal
death due to hepatic insufficiency
