Guys now I can briefly give you the ministerial structure of Tobacco Control. We have had two offices dealing with Tobacco Control from the onset of the Act in 2007.
1. Tobacco Control Board: Creation of Section 5 of the Act and the Secretary is the Head of Environmental Health meaning that board secretariat sits in this Division. This is where I served as secretariat upto 2023 when I handed over to a PHO who is still there upto now. No Medical Officer.
2. There was Tobacco Control Division which was headed for along time by a Clinical Officer from 2007 upto 2023 when it metamorphosed into Drugs and Substance Abuse Division headed by a Medical Doctor.
3. OHS: We wrestled this from Division of Non Communicable Diseases in 2013 when it was given to a Medical Doctor and brought it back to Environmental Health again in 2022 they took it to Standards Directorate and we brought it back to EH division. I equally handed it over to a PHO in Environmental health Division. Currently we are represented at DOSH by a PHO who took over from me.

So let us now engage from a point of understanding. Those who said we are not mentoring and we are not working with PHOs and even mentioned that we edged some PHOs out be appropriately informed. I left the Division intact with all that I was doing. Blames aside you can engage me on other things.

Our union honoured an invite with the County Government of Baringo and held serious deliberations in regard to our demands highlighted in our strike notice.

The following are the quick wins:
1. A total of 150 PHOs to be hired at the elapse of the next financial year.
2. A total of 6 PHOs to be replaced immediately.
3. The current Director, Public Health stepped aside to pave way for competitive recruitment and filling of the said position by a licenced & competent PHO.
4. Mass promotions of all eligible PHOs/Ts in the common cadre establishment to be done immediately.
5. Promotions in the competitive establishment to be done immediately.
6. A deliberate plan to ensure seamless recruitment of PHO/Ts put in place.
7. Redesignations and right placement to be done immediately.

We take this great opportunity to thank our CPHO Baringo, KEHPHPU Baringo Branch leadership and all our colleagues for standing firm and staying put.

He stands in everyone’s way, creating unnecessary bureaucratic bottlenecks that compromise efficiency and effectiveness in healthcare delivery.

This issue is not new; it is part of a long-standing cadre war instigated by the DG, which we previously presented to the task force. Unfortunately, our concerns were not reflected in their report, further raising doubts about the task force’s impartiality.

This is the first time since independence the government has taken the right direction in addressing the issue of diseases through preventive and promotive health at all levels across the 47 county governments. Though the government has put up many health facilities and trained many medics and middle level health workers, the disease burden has been on the upsurge trend.

This is due to focus on only curative medicine. Notably, 85% of the communicable diseases can be prevented at the community level through preventive interventions and thus save the government substantial finances remitted to healthcare system in form of exchequer. Let us not lose gains so far that have been made through the establishment of the state department of public health and professional standards which so far has performed well.

Lastly, given these grave concerns including misleading the taskforce by the DGs actions, we urge His Excellency the President Dr. William Samoe Ruto to urgently intervene and take appropriate action to, Restore fairness, inclusivity, and accountability in the health sector. Ensure that preventive healthcare remains a national priority and is not sabotaged by internal power struggles. Hold those responsible for misinformation and inefficiency accountable, particularly the DG, whose actions threaten the progress made in public health. His decisive action will protect the integrity of public health governance and ensure accountability within the Ministry of Health

We wish to respond to the allegation presented by the presidential taskforce that appeared on the dailies on 4th February 2025.

All of the technical shortfalls found by the taskforce were due to inefficiency of the Director General of Health (DG) as a technical advisor at the Ministry.

The alleged conflict between the two Principal Secretaries (PSs), which was in any case not part of the terms of reference for the taskforce, was given a lot of emphasis as a diversionary to blindfold technical failures by Director General. However, this claim has been thoroughly investigated and found to be entirely unsubstantiated. It is now evident that this narrative was fabricated by an insider within the taskforce, an individual based at Afya House—whose motivations appear to be driven by personal interests rather than the well-being of the health sector.

Furthermore, the composition of the task force raises serious concerns about its objectivity. It was made up entirely of curative healthcare cadres, with no representation from the preventive cadres. Further the director general who interested part should not have been the member of the taskforce. In the National Taskforce on Police Reforms led by Justice Maraga, the commissioner of prison and inspector general of police were not members of the taskforce since they are interested parties, they gave their submissions just like other many stakeholders. These imbalances in the Health Human resources taskforce undermines the credibility of its findings and recommendations.

We wish to reiterate that the purported disagreement between the two departments within the Ministry of Health was nothing more than baseless rumors. Our verification confirms that the two PSs work in close consultation on a daily basis and maintain a professional and friendly relationship.

Additionally, we recognize that the issue of cadre infights marginalization was initiated by the current Director-General (DG) long before the present administration took office. This individual has consistently demonstrated lack of teamwork, actively blocking preventive healthcare cadres from engaging with the Cabinet Secretary (CS). In fact, the former CS left office without ever granting the 18 professional cadres a courtesy consultation meeting.

Recently, the DG called for training on disease surveillance but deliberately excluded all preventive health cadres, including port public health officers and county public health officers’ disease surveillance teams, who are the actual and critical implementers of these programs. This exclusion was clearly an attempt to undermine their role in preventive health which is the cornerstone of this administration. It is evident that the DG took advantage of his position in the task force to settle personal scores.

This raises a fundamental question; did the task force conduct a proper fact-finding mission by engaging the PSs on the alleged conflict, or was its report purely based on rumor-mongering to serve personal selfish interest of Director General for Health? It is becoming increasingly clear that the DG orchestrated this entire process to maliciously dismantle the Department of Public Health and Professional Standards, an entity established by the President through an executive order to support the successful implementation of Universal Health Coverage (UHC) across the country. The DG’s ultimate goal appears to be the consolidation of power under his office, allowing him to mismanage the department without oversight.

Moreover, this is a person who remains inaccessible after working hours during emergencies, he does not pick calls from some cadres selectively, a critical failure for someone in his position. This has directly affected service delivery, as his leadership style is characterized by obstructionism, poor decision-making, and unilateral actions that hinder the smooth operation of the ministry.

Undiagnosed disease - Democratic Republic of the Congo: Situation at a glance

Between 24 October and 5 December 2024, Panzi health zone in Kwango Province of Democratic Republic of the Congo recorded 406 cases of an undiagnosed disease with symptoms of fever, headache, cough, runny nose and body ache. All severe cases were reported to be severely malnourished. Among the cases, 31 deaths have been registered. The majority of cases reported are among children, particularly those under five years of age. The area is rural and remote, with access further hindered by the ongoing rainy season. Reaching it from Kinshasa by road takes an estimated 48 hours. These challenges, coupled with limited diagnostics in the region, have delayed the identification of the underlying cause. Rapid response teams have been deployed to identify the cause of the outbreak and strengthen the response. The teams are collecting samples for laboratory testing, providing a more detailed clinical characterization of the detected cases, investigating the transmission dynamics, and actively searching for additional cases, both within health facilities and at the community level. The teams are also aiding with the treatment of patients, risk communication and community engagement. Given the clinical presentation and symptoms reported, and a number of associated deaths, acute pneumonia, influenza, COVID-19, measles and malaria are being considered as potential causal factors with malnutrition as a contributing factor. Malaria is a common disease in this area, and it may be causing or contributing to the cases. Laboratory tests are underway to determine the exact cause. At this stage, it is also possible that more than one disease is contributing to the cases and deaths.

The “Epuka Uchafu, Afya Nyumbani” initiative is being launched at Majengo Mapya Village in Bangladesh, Mikindani Ward, Jomvu Sub-County. This is in response to growing undesired methods of waste management and disposal. The initiative represents a crucial step in Kenya's mission to enhance cleanliness and public health. Kenya generates approximately 22,000 tonnes of waste daily, only 10% is properly managed, and 7.3% of households still lack basic sanitation facilities.

The new initiative focuses on empowering communities to create sustainable, locally-driven solutions for waste management and sanitation.
This initiative involves;
1. Waste management at the household level,
2. Food and personal hygiene,
3. Environment sanitation,
4. Water safety / treatment,
5. Cleaning,
6. Conservation of natural resources via tree planting.

The initiative will ensure a healthier, safer future for all Kenyans by empowering communities to take charge of their environment and well-being. The County Public Health Officer encouraged the community members to observe proper hygiene at household levels urging the Community Health Promoters (CHPs) to be the role models of this initiative. The Sub-County MoH emphasized proper personal hygiene, oral hygiene, food hygiene, and handwashing to prevent diarrheal diseases.

The team managed to plant 150 mangrove seedlings. The public health officers distributed 10,000 tablets of aqua tabs and demonstrated how to treat water at the household level to prevent waterborne infections like cholera and diarrhoea. Also, the PHOs distributed 600 sackets ORS to community members to be used to prevent dehydration in children under 5 years in case of diarrheal infections.
