5️⃣ Neurogenic Shock | الصدمة العصبية 🧠.

تكون بسبب حدوث تلف في الجهاز العصبي المركزي (Sympathetic) ، عادة بسبب إصابة في الحبل الشوكي، ينتج عنه تمدد بالأوعية الدموية > يقل الدم الواصل للاعضاء الحيوية وبالتالي ممكن يحدث > فقدان وعي للمريض ومشاكل في الدماغ.

Sign and symptom | 🤢

- Hypotension 🔻
- Bradycardia 🔻
- Weak pulse and cold skin 🥶
- Difficulty breathing.

Treatment | 💊

Fluid resuscitation and vasopressor ⚠️.

4️⃣ Anaphylactic Shock | الصدمة التحسسية

تحدث نتيجة التحسس الشديد وهي ردة فعل الجهاز المناعي للاجسام الغريبة.

ومن أسبابها بعض انواع الادوية، المأكولات مثل البيض او المكسرات .. وتوجد اسباب أخرى متعددة.

Signs and symptoms | 🤢

- Swelling of the lips, tongue, face and eyes.
- Tachycardia ⤴️
- Difficulty breathing and speaking.

Treatment | 💊

تعتبر حالة طارئة لأنها تسبب ضيق بالنفس بالتالي تحتاج تدخل سريع، ويكون علاجها ( Epinephrine Injection💉 ).

3️⃣ Cardiogenic shock | صدمة قلبية

تكون المشكلة الاساسية هي ان القلب لا يضخ الدم الكافي للاعضاء > ضعف في التروية ( tissue perfusion ).
واحد اكثر الأسباب لهذه الحالة ( Cardiac Attack )

Signs and symptoms | 🤢

- Hypotension ⤵️
- Chest pain.
- Dyspnea.
- Abnormal heart rate.

Treatment | 💊
- Drug of choice (Dopamine).
- Respiratory support.

2️⃣ Septic shock | الصدمة الإنتانية

تحدث بسبب عدوى في مجرى الدم وتكون عادة عدوى بكتيرية 🦠.

نتيجة لهذه العدوى يقوم الجسم بالدفاع ويحدث تجمع لكرايات الدم البيضاء + عوامل الالتهاب (Pro-inflammatory cytokines) > مسببًا توسع الأوعية الدموية بالتالي انخفاض بضغط الدم.

Signs and symptoms | 🤢

- Hypotension ⤵️.
- ⁠cold extremities 🥶.
- ⁠Confusion.
- ⁠High WBCs (over 10,000) ⤴️.
- ⁠High or very low Temp (96 F).

Treatment | 💊
- Drug of choice > (Norepinephrine)
- Respiratory support.
- IV fluids to normalize blood pressure.
- Antibiotics.

1️⃣ #Hypovolemic_shock

يندرج تحتها

- Hemorrhagic ( Bleeding, Vomiting and diarrhea.. )

- Non-hemorrhagic (Burn, renal loss.. )

الاثنين راح يعاني فيها المريض من فقدان سوائل بالجسم ينتج عنه > انخفاض الضغط > زيادة النبضات ( لان القلب يحاول يضخ كمية دم عالية للتعويض ' Compansated mechanism ' ) بالتالي بسبب انخفاض ضغط الدم تقل التروية والاكسجين الواصل لباقي اعضاء الجسم ( low tissue perfusion ) .

Sign and symptom | 🤢

- Hypotension ⤵️
- Tachycardia ❤️⬆️
- Diaphoresis 😰
- Cold extremities 🥶

Treatment | 💊

Early resuscitation ⚠️
بعد معرفة السبب الأساسي، غالبًا نبدًا نعوض كمية الـ سوائل الي فقدها المريض.

Different type of #shock ⚠️

بالبدايه ايش هو تعريف الـ Shock = عبارة عن حالة يدخل الجسم فيها لصدمة لعدم وجود تروية كافية لأعضاء الجسم ! 🩸

الاختلاف بانواع الـصدمات يكون باختلاف السبب فقط ونقدر نميزه من خلال الاعراض والتحاليل.

والهدف الاساسي لعلاج حالات الـ Shock هي الحفاظ على مستوى ضغط الدم الطبيعي ثم علاج السبب الأساسي ⚠️.

ببساطة عباره عن خطوات/تدخلات نطبقها لمنع العدوى المرتبطة بـ ( القسطرة البولية، القسطرة وريدية، جهاز التنفس الصناعي )

1️⃣ #CAUTI ( Cathert Association Urinary Track Infection ).

خطوات منع العدوى المرتبطة بالقسطرة البولية ؟

- Hand wash.
- Insert under aseptic technique.
- Review urinary catheter necessity daily.
- Keep the urine bag below the bladder level.
- Perineal care.
- Remove the catheter if no need it!

2️⃣ #CLABSI ( Central Line Association Blood Stream Infection ).

خطوات منع عدوى مجرى الدم المرتبطة بالابرة/القسطرة المركزية |

- Daily assess of nessicty.
- Insert under aseptic technique.
- Change dressing as per protocol.

3️⃣ #VAP ( Ventilator associated pneumonia ).

خطوات لمنع التهاب الرئة الناتج عن جهاز التنفس |

- Elevate head of the bed 35-45%
- DVT and GIT ulcer prophylaxis
- Oral care with chlorhexidine q4hr
- Daily sedation vaccination and weaning trial.

اللى ما تعبر المشيمة و تضرر البيبي زي 👇🏻

أما بقية الأدوية المخدره اللى ما نقدر نعطيها لانها راح تعبر المشيمة و تسبب له مشاكل زي 👇🏻

● Benzodiazepines (valium)

Sedation
لتقليل درجة للوعي، بحيث انه تصبح استجابة المريض للمحفزات الخارجية محدودة.

Minimal Sedation | = Anxiolysis

يستخدم غالبًا لتخفيف القلق عند المريض وتأثير جدًا ضئيل على الوعي.

Moderate | = Conscious sedation

يقلل من وعي المريض مع وجود استجابه للمؤثرات الخارجية (Verbal and Tactile stimuli)

Deep |

استجابة المريض تكون فقط للمؤثرات المؤلمة والمتكرره. (Painful stimuli)
