IRRIGATION
---------------------------

◾ أيه هو ال irrigation ؟؟
➖فكرة الاريجيشن إنى أنضف الكانال ( washing out ) من أى انفكشن باستخدام medicated fluid ..

◾ الهدف من أى endodontic treatment  عموما بينقسم لجزئين :

◾ Mechanical & Chemical  :

▶ flushing ,

▶ Lubrication ,

▶ Dissolution ,

▶ Smear layer removal .

◾ Biological :

▶ Anti-microbial effect .

عمليه ال Cleaning & Shaping  دى تعتبر
Chemo-mechanical preparation

▶ Chemical

▶ Irrigants
Intacanal medicament

▶ Mechanical

◾Instruments

◾ الشروط اللى لازم تكون متوفرة فى اى irrigant :

1▶ Organic & InOrganic tissue solvent ,

2▶ Anti-microbial action ,

3) Non-toxic ,

4▶ Low surface tension ,

5▶ lubricant ,

6▶ Non-irritant of periapical tissue ,

7▶ Not cause tooth staining .


◾نيجى بقى للجزء الأهم فى الموضوع ..

◾أشهر أنواع ال irrigants :

▶Sodium Hypochlorite :

أشهر نوع أريجيشن على الإطلاق ..

◾ Advantantages :

➖ Mechanical flushing of debris from the canal ,

➖ tissue dissolving capacity ,

يعنى بيدوب أى tissue موجود داخل الكانال سواء كان
vital or non-vital

➖Antimicrobial action ,

➖lubrication ,

➖ Inexpensive ,

➖ easily available .

➖ تركيزه يتراوح بين ( 0.2 - 5.25 % )
ولكن أفضل تركيز نستخدمه هو ( 2.5 % )

🔥مع العلم إن فاعلية الصوديوم هيبوكلوريت بتزيد لو سخنا ال solution

🔴ملحوظة

نقطة مهمة جداا هنا لازم نعرفها لانها ممكن تحصل مع أى حد فينا .. بس المهم إنى اعرف كيف اتعامل لو حصلت .. هى  Sodium Accident ..

طيب أيه معناها اصلا ..

هذه ببساطة كده يعنى وانا بعمل اريجيشن شوية منه خرجوا وعدوا ال apex نتيجة ضغط ال injection  داخل الكانال ..

طيب هشوف ايه على البيشنت لو حصلت ؟

◾ Signs & Symptoms :

➖ pain & swelling ,

➖ Haematoma ,

➖Hemorrhage ,

➖ inflammation of the gingival tissue .

◾ علشان اتجنب حدوث المشكله دى ..

➖ لازم نخلى ال irrigation needle تكون loose داخل الكانال ..

➖وكمان مش لازم أبدا استخدم القوة وأنا بعمل irrigation ..

◾ Disadvantages :

➖ Toxic

➖ ineffective at low conc.

➖ unpleasant odor

➖ineffective in smear layer removal .

◾Chlorhexidine :

➖ has broad spectrum of antimicrobial action ..

يفضل استخدامه بتركيز 0.12 - 2.0 %

◾Advantages :

➖effective on awide range of bacterial strains ..

➖ less toxicity ..

➖ NaOCl + CHx 🔜 🔝 Effectiveness ..

➖allergic action to CHx is rare ..

◾ Disadvantages :

➖ lack of tissue dissolving capacity ..

➖ Inability to remove the smear layer ..

◾  Hydrogen peroxide H2o2 :

➖ Oxidizing agent ..

➖ almost used in conjunction with NaOCl ..

لما بستخدمه مع NaOCl  بيحصل فوران اللى بدوره بيساعد فى خروج أى debris بره الكانال ..

◾ Advantages :

➖ produce less damage to periapical tissues ,

➖good antimicrobial agent ,

➖when used with CHx , it disloges the smear layer ..

➖cause bleaching which is beneficial disadvantage ..

◾ Normal saline :

افضل اريجيشن فى حالات الابسس ومع الاطفال فى حاله الخوف من بلعه ..

◾ Advantages :

➖biocompatibility ,
that mean no adverse effect when extruded periapically because osmotic pressure the same as that of blood

◾ Disadvantages :

➖No disinfection properties ,

➖can't remove microbial flora of accessory canal ,

➖No antimicrobial activity ,

➖ Doesn't remove smear layer
