MEDIAL GASTROCNEMIUS TENDONOSIS: Intermediate increased signal medial gastrocnemius tendon from mild tendonosis. Not uncommon to see & more common than at the lateral gastrocnemius tendon insertion. However, not sure how symptomatic it can be.

FEMORAL CONDYLE POSTERIOR CARTILAGE LOSS Full thickness cartilage (Pink arrow) loss with subcortical bone marrow oedema (Orange arrow). The posterior cartilage I find is much better assessed on sagittal scans than axial.

Not a difficult diagnosis but more a matter of moving our gaze to look posteriorly specifically on the sagittal scans to not miss it.

PROXIMAL INTERSECTION SYNDROME: Typical changes seen in Proximal Intersection Syndrome at crossover of Compartment 1 tendons over Compartment 2. Tenosynovitis, tendonosis and adjacent soft tissue inflammation. No adjacent bone marrow oedema which can be seen if it's severe enough.

We have just successfully completed our 2-day Onsite Wrist MRI guided Mini Fellowship in KL last weekend (pictures coming) but we have a new Online Wrist and Hand MRI Minin Fellowship coming in March 2025. DISPLACED FLAP TEAR

Displaced meniscal tissue (Pink arrows) deep to the superficial MCL (Green arrow). Flap tear of the medial meniscus body.

This is a horizontal tear through the body and the inferior portion has flipped out (Pink arrows) of the joint and lies in the recess between the superficial MCL (Green arrow) and tibia. The residual body (Blue arrow) in the joint is small reflecting loss of meniscus.

MORTON'S NEUROMA

Nodular low signal structure on T1 and T2FS on the plantar side of the 2nd interspace. Features are of a MORTONS NEUROMA. Important to always assess the plantar plate to ensure that there isn't a plantar plate tear with nodular granulation tissue/fibrosis mimicking a neuroma.

Complete rupture of the Biceps tendon from the radial tuberosity (Blue arrow). The subcortical cystic changes in the radial tuberosity (Pink arrows) are enthesopathic changes indicating a chronic enthesopathy prior to the rupture.
