Adenoid hypertrophy –
Adenoids are present at birth but not completely developed. Begin to enlarge by 2 years of age, continue till 6 years, atrophy starts at puberty age and disappear by 20 years of age. Presentation of the child will be with nasal obstruction, mouth breathing and snoring. Due to mouth breathing, high arched palate formed and crowding of teeth occurs. Snoring is due to palatal flutter (when adenoid enlarges, airway is narrowed and velocity of air increases which causes vibrations of soft palate). Enlarged adenoid also obstructs both Eustachian tube openings and cause serous otitis media. If infection of this middle ear fluid occurs, then acute otitis media sets in. Overall facial changes that appear are pinched up nose, absent nasolabial crease, open mouth, crowded teeth and a dull look – all these called οƒ  adenoid facies. X-ray nasopharynx lateral view can show adenoids and their grade of hypertrophy. Medical Rx initially – antibiotics, antihistaminics, decongestants. If recurrent episodes, then adenoidectomy.
