Several individuals present with explosive watery diarrhea, mild fever, and profound fatigue. Epidemiologically, the common link is the eating of imported salad greens. Acid-fast preparations of fresh stools reveal large oocysts. Furthermore, the illness responds to TMP–SMZ treatment. What is the most likely pathogen?

📕Imported raspberries from Central America have also been linked to cluster cases of cyclosporiasis in the United States. More than likely the use of contaminated river water to wash the berries or to apply fungicide and insecticide prior to shipping is a contributing factor. Cyclosporan oocysts are noticeably larger than those of Cryptosporidium. Although both Cyclospora and Cryptosporidium have oocysts that are acid-fast, the sensitivity to the antimicrobial TMP–SMZ is the distinguishing attribute.

A25-year-old man with acquired immune deficiency disease (AIDS) has malabsorption, chronic abdominal pain, low-grade fever, and nonbloody diarrhea. In fecal smears, large numbers of oocysts containing four sporozoites were observed. Which of the following organisms is most likely the etiological agent?

🎯MICROBIOLOGY MCQ & NOTES 🎯 🔬🧬🦠🧫📚🎯

🔥All five organisms are protozoa. There are two intestinal protozoa specifically associated with  AIDS that can cause transient diarrhea in immunocompetent individuals, but can cause de bilitating, and potentially life-threatening, chronic diarrhea in  AIDS patients. These organisms are  C. parvum with no effective drug therapy, and Isospora belli treated with folate antagonists, such as trimethoprim-sulfamethoxazole.

🔥 Microsporidia may cause severe, persistent, watery diarrhea in  AIDS patients, but they produce spores rather than oocysts (choice D).

🔥  T. solium is the pork tapeworm which occasionally may cause diarrhea (choice E).

🔥 E. histolytica and G. lamblia may cause diarrhea, but they are not closely associated with  AIDS (choices B and C).

This organism, a frequent skin colonizer, can contaminate food during preparation. The food is then left at room temperature for several hours during which time the bacteria replicate and secrete heat-stable enterotoxins. The toxins enter the blood and affect the vomiting control center of the brain, inducing vomiting 1 to 6 hours following ingestion of contaminated foods. Settings at risk for outbreak of staphylococcal food poisoning include group picnics, restaurants, and school cafeterias or other situations in which large numbers of people eat food made in batches by food handlers, colonized with the bacteria, who do not use precautions to prevent contamination. Foods that are most frequently implicated are those that are high in salt or sugar, such as salted meats, ham, potato or macaroni salad, and cream-filled pastries or pies. C. jejuni causes a diarrheal illness and is associated with the consumption of undercooked chicken. C. perfringens also causes a diarrheal illness with an incubation period of 8 to 14 hours following ingestion of contaminated food. B. cereus causes vomiting and is most frequently associated with the consumption of fried rice. Salmonella food poisoning causes a bloody diarrhea 16 hours to 2 days after ingestion of undercooked chicken, eggs, or other contaminated foods
