βœ… First Heart Sound: S1 

Heard upon closing of the mitral & tricuspid valves.  

Sound produced by closure of mitral valve is termed M1, and closure of tricuspid valve is termed T1. M1 is louder than T1 due to higher pressures in the left side of the heart. M1 radiates to all cardiac listening areas (loudest at the apex). T1 is usually only heard at the left lower sternal border. M1 is the main component of S1.  

🩺 Key: Split S1 heart sound is best heard at the tricuspid listening area, as T1 is much softer than M1.  

M1 sound occurs slightly before T1 even though both valves close simultaneously, only a single heart sound is heard. But in some cases, of normal patients, a β€œsplit S1” can be heard. It happens when mitral valve closes significantly before tricuspid valve, so each valve creates a sound.  

βœ… Second Heart Sound: S2  

Heard upon closing of the aortic & pulmonic valves. 

Sound produced by closure of aortic valve is termed A2, and closure of pulmonic valve is termed P2. A2 sound is louder than P2 due to higher pressures in the left side of the heart. A2 radiates to all cardiac listening areas (loudest at the right upper sternal border). P2 is usually only heard at the left upper sternal border. A2 is the main component of S2.  

🩺 Key: Split S2 heart sound is best heard at the pulmonic valve listening area, as P2 is much softer than A2. S2 is split in 90% of people.

βœ… Overview

Heart sounds are produced from a specific cardiac event such as a closure of a valve or the tensing of the chordae tendinae. Main heart sounds are S1 & S2. The S3 can be normal, at times, but may be pathologic. The S4 is always pathologic. Closing of the valves causes audible sounds. No audible sounds occur when the valves open.
’Lub’ is first heart sound S1 & associated with closure of Atrioventricular valves at the beginning of the systole. ’Dub’ is second heart sound S2 & associated with closure of Semilunar valves at the end of systole

*Important: Heart sounds are discrete, short audible events from a specific cause. Heart murmurs are abnormal EXTRA heart sounds that are heard during a cardiac cycle produced by turbulence of blood flow through the heart and its valves.

A study in the US found women burn more body fat during morning exercise, whereas evenings count more for men.

Researchers say differences in hormones, biological clocks and sleep-wake cycles between the sexes, could all play a role.

Much of what is known on this topic is based on studies on men, the scientists say.

The 12-week study monitored the effects of a varied fitness programme and a specially-designed meal plan.

All those who took part in the study improved their overall health and performance, no matter when they exercised but there did appear to be differences between fat-burning and muscle strengthening exercise.
