What is a positive- or a negative-polarity virus ?

- Both terms are usually applied to RNA viruses ..

- positive-polarity ( +RNA ) means that its genome ( RNA ) is translated directly into proteins.

- negative-polarity ( -RNA ) means that the genome ( RNA ) needs to be transcribed into a +RNA to be then translated into proteins.


These can also be called as positive-sense or negative sense RNA viruses.

اهم الملاحظات من مصدر ديفدسون بما يخص محاضرة الميدسن الثانية و يُفضل تضيفوها لملازمكم لان الدكتور كال داخلة :

- in infective endocarditis , the tricuspid valve is usually affected .

- infective endocarditis a common complication of Intravenous drug use.

- the most susceptible areas to infective endocarditis are site of endocardial damage as a result of high-pressure jet of blood as in ventricular septal defect , mitral regurgitation , aortic regurgitation , etc..

- PATHOPHYSIOLOGY of infective endocarditis: endocardial damage >> platelet and fibrin depositions >> these deposition will be colonised by blood-borne organisms e.g. staph aureus >> an infection is established >> inflammatory processes initiated.

- bullous impetigo is usually caused by exfoliatin toxin that cleaves desmoglein-1 ( a protein involved in desmosomes ) >> superficial epidermal split with blister formation.

- ecthyma is considered as a deep version of impetigo.

- both bullous impetigo and ecthyma are treated with mupirocen or fusidic acid in mild cases , but with clarithromycin or flucloxacillin in severe cases.

- in superficial folliculitis , the primary lesion is follicular pustules and erythema.
And it’s usually self-limiting and pustules resolve without scarring

- deep folliculitis could be a furuncle ( boil ) or a carbuncle and both types resolve by their rupture discharging pus and leaving a scar.

- SSSS ( staphylococcal scalded skin syndrome ) results from systemic circulation of exfoliatin toxin , the same toxin that causes impetigo.

- presentation of SSSS is usually severe , but with rapid recovery and absence of scarring.

Include
Low grade fever , cough , haemoptysis , dyspnea , chest pain , bronchitis and brochopneumoniae “inflammation of bronchioles “

And most importantly is accumulation of eosinophils in the lung 🫁

Loeffler's is allergic pneumonitis condition not ، pneumonia !
