The NADAS Criteria refer to a scoring system developed by Dr. Arthur Nada in 1971 for evaluating the severity of congenital heart disease (CHD) in children. The NADA's Congenital Heart Disease Severity Score is used to assess the risk associated with various congenital heart defects based on specific clinical and hemodynamic factors.
NADAS Criteria Scoring System
Each factor is assigned a numerical value, and the total score helps determine the severity of the condition.
Factors Considered in the NADAS Criteria:
Cyanosis (Hypoxia)
Mild (e.g., transient cyanosis) → 1 point
Moderate (cyanosis at rest) → 3 points
Severe (profound cyanosis) → 5 points
Pulmonary Blood Flow
Normal → 0 points
Increased (e.g., left-to-right shunt) → 1 point
Decreased (e.g., Tetralogy of Fallot) → 3 points
Pulmonary Hypertension (PH)
Absent → 0 points
Mild → 1 point
Moderate → 2 points
Severe (mean pulmonary artery pressure > 50 mmHg) → 3 points
Left Heart Obstruction
Mild (e.g., small aortic stenosis) → 1 point
Moderate → 2 points
Severe (e.g., hypoplastic left heart syndrome) → 3 points
Right Heart Obstruction
Mild → 1 point
Moderate → 2 points
Severe (e.g., severe pulmonary stenosis) → 3 points
Aortic Arch Abnormalities
Mild (e.g., isolated coarctation) → 1 point
Moderate → 2 points
Severe (e.g., interrupted aortic arch) → 3 points
Cardiac Failure (CHF)
Absent → 0 points
Mild → 1 point
Moderate → 2 points
Severe (requiring ICU care) → 3 points
Interpreting the Score
0–2 points: Mild congenital heart disease
3–4 points: Moderate congenital heart disease
5+ points: Severe congenital heart disease
This system helps guide clinical decision-making, prognosis, and management strategies for children with CHD. However, modern imaging techniques and classification systems have refined risk assessment beyond the original NADAS criteria.
