On January 29, 2025, a Bombardier CRJ700 airliner, American Airlines Flight 5342, and a United States Army Sikorsky UH-60L Black Hawk helicopter, Priority Air Traffic 25, collided mid-air over the Potomac River, about half a mile short of runway 33 at Ronald Reagan Washington National Airport in Arlington, Virginia.
Takeaways:
1. Inexperienced Crew – Both the Pilot in Command (PC) and Pilot (PI) had insufficient flight hours for conducting an annual check ride in dense Class C and Restricted airspace.

Having two crew chiefs would have improved hazard detection and potentially prevented the crash. It is unclear (or has not been released) what side the crew chief was found sitting on.
6. ATC Miscommunication – The UH-60 crew missed or misinterpreted tower instructions, failing to maintain clearance behind and below an oncoming CRJ flight.
7. Cockpit Limitations & ADS-B Uncertainty – The UH-60L had an older analog cockpit and an ADS-B system that should have automatically activated via the WoW switch upon leaving the ground, but investigators cannot confirm if it was transmitting.
8. CRJ’s Course Change & Last-Minute Evasion – The CRJ initially planned to land on Runway 1 but opted for Runway 33 upon ATC prompt, putting it on a collision course with the UH-60.
9. The CRJ's Traffic Collision Avoidance System (TCAS) warning alerted them to traffic but gave no evasive maneuvers guidance due to the CRJ's altitude (ie. being below 500FT AGL). The CRJ crew saw the UH-60 at the last second and attempted an evasive climb and roll, but was too late to avoid the crash.
