*Systems of Medication Measurement*

Medication measurement systems are crucial for accurate dosing to ensure patient safety and therapeutic efficacy. The three primary systems used are the Metric System, the Apothecary System, and the Household System.

1. Metric System

The metric system is the most commonly used system of measurement in healthcare due to its accuracy and ease of conversion. It is based on multiples of ten, making it straightforward for calculations.

Units of Measurement

Volume: Liter (L), milliliter (mL)

1 L = 1000 mL

Weight: Gram (g), milligram (mg), microgram (mcg), kilogram (kg)

1 kg = 1000 g
1 g = 1000 mg
1 mg = 1000 mcg

Length: Meter (m), centimeter (cm), millimeter (mm)

1 m = 100 cm
1 cm = 10 mm

Conversions

To convert larger units to smaller units, multiply by 1000.

Example: 1 g = 1000 mg

To convert smaller units to larger units, divide by 1000.

Example: 1000 mcg = 1 mg

Practical Examples

A medication order for 500 mg of Drug A could be prepared by calculating the amount based on the concentration, e.g., if Drug A is 250 mg/mL, then 500 mg would be 2 mL.
Infusions may use mL/hr (milliliters per hour) to determine IV drip rates.

2. Apothecary System

The apothecary system is an older system that is less frequently used today but may still be encountered in specific contexts such as compounding pharmacies or older prescriptions.

Units of Measurement

Volume: Minim (m), fluid dram (fl dr), fluid ounce (fl oz), pint (pt), quart (qt), gallon (gal)

1 fl oz = 8 fl dr
1 pt = 16 fl oz
1 qt = 2 pt
1 gal = 4 qt

Weight: Grain (gr), scruple (s), dram (dr), ounce (oz), pound (lb)

1 scruple = 20 gr
1 dr = 3 scruples
1 oz = 8 dr
1 lb = 12 oz (apothecary) or 16 oz (avoirdupois; more commonly used in the U.S.)

Conversions

Minim to mL: 1 minim = 0.06 mL
Fluid dram to mL: 1 fl dr = 3.69 mL (often rounded to 4 mL)
Fluid ounce to mL: 1 fl oz = 29.57 mL (often rounded to 30 mL)
Grain to mg: 1 gr = 64.8 mg (often rounded to 65 mg)

Practical Examples

A prescription might call for a medication to be given in grains (e.g., 5 gr of aspirin). Converting to metric, 5 gr = 325 mg.
Fluid measurements might need conversion, such as 1 fl oz of a liquid medication being about 30 mL.

3. Household System

The household system is commonly used for over-the-counter medications and when educating patients about dosages for home use. Its measurements are not as precise as the metric system and are generally used for non-critical measurements.

Units of Measurement

Volume: Drop (gtt), teaspoon (tsp), tablespoon (tbsp), cup

1 tsp = 5 mL
1 tbsp = 15 mL
1 cup = 240 mL

Weight: Pound (lb), ounce (oz)

1 lb = 16 oz
For household use, weight is typically less precise and used mainly for larger quantities.

Conversions

Teaspoon to mL: 1 tsp = 5 mL
Tablespoon to mL: 1 tbsp = 15 mL
Cup to mL: 1 cup = 240 mL

Practical Examples

If a liquid medication is to be given as 1 tsp, you would dispense 5 mL.
Patient education might involve converting medication doses from milliliters to teaspoons or tablespoons for easier administration at home.

Important Considerations:

Accuracy: The metric system is the most accurate, followed by the apothecary system, while the household system is the least accurate and subject to individual variation.
Context: Metric measurements are standard
